I don’t have an account; how do I upload my first case?

  • At the “Log in” screen, click the “Sign Up” button.
  • Please provide us with the following information:

    • Hospital Address:
    • Hospital Phone:
    • Hospital Email: (this email will get a notification when a report is uploaded to our Reports website, so it should not be a personal email if possible-should be the hospital one)
    • Billing Contact:
    • Billing Phone Number:
    • Billing Email Address:
  • After we receive your information, we will set you up with an account and provide you with a username and password to start submitting cases.
